A & R Request for Transfer

There are two forms available to request a Transfer.  The first form allows the Union to assist you in finding a "match" and the second is the "State Transfer Form" so you can get on the official transfer list

We will have a fill-able online version of the Union "Request for Transfer Form" available soon.  Until then you can download the form in MS Excel Format (18KB).

Since completion of this A&R transfer request DOES NOT eliminate the need to complete the DAS transfer request we have provided that form for you to download and complete as well in MS Word Format.

Requirements:

1. Must have permanent status in class title

2. Title for title swap or comparable title by DAS; case by case basis for unusual circumstances

3. PPT (Pre-professional Trainee) excluded until permanent appointment to class title

4. Managers in an A&R title eligible to apply; however, all A&R members will receive top priority

5. Transfers will be based on total time within current class title

6. Return completed request to the A&R Union (via fax or U.S. mail)
